Applications are now being accepted for Early Head Start at the Appletree Academy Child Care Partnership (CCP) located at 9 Meadow Lane in Decatur AL.

Parents must bring to the appointment:

• Child's birth certificate
• Verification of income for 2017 (W-2 form, check stub, Social Security statement, child support, unemployment, etc.)
• Medicaid or insurance card
• CMA card or waitlist letter

Additional information needed, if applicable:

• WIC
• Food stamps (SNAP)
• College ID
• MIlitary ID
• Proof of Kinship Care
• IFSP

Services are provided at NO COST to families.  

Provided by the program during school hours:

• Pull-ups/diapers
• Wipes
• Formula
• Baby food
• School supplies

Homeless families and infants/toddlers with special needs receive priority points
