A Madison area Intake Day is scheduled for Thursday, April 20th from 8 a.m. to 2 p.m. at the Madison Office, located at 101 Pension Row in Madison.  Applications for Early Head Start (birth to 3 years of age, including pregnant mothers) and Preschool Head Start (ages 3 to 4 years) will be accepted for the 2017-18 school year for classrooms in the Madison area including:

• Dawson
• Farley
• Harvest
• Madison Premier Pre-school
• MLK
• Premier Pre-school
• Rolling Hills
• UAH Early Learning Center
• Whitesburg
• Williams

You must bring the following information to the intake site to complete the application process:

• Child’s Birth Certificate or other form of proof of birth
• Proof of Income (Tax Return, W2, Pay Stub, Letter from Employer, SSI statement, Social Security Statement)—Last Calendar Year or Last 12 Months
• Child’s Blue Slip
• Child’s Medicaid/Insurance Card
• Proof of Disability (IEP or IFSP)
• Social Security Numbers for all Family Members
• WIC/Food Stamps Documentation
• Name, Address and Phone # of Child’s Doctor and Dentist
• Student ID if parent(s) enrolled in College
